Amergis Healthcare Staffing is seeking a Case Manager RN (Contract - Full Time) in Jeffersonville, IN. The role coordinates continuum of care for assigned patients and ensures efficient use of resources and adherence to care plans.

The ideal candidate holds active RN licensure in Indiana, with 1+ year of case management experience preferred and CPR as applicable. You will work to meet regulatory requirements and deliver high-quality patient care.

Qualifications

• Current RN licensure in state practicing.
• At least one year of Case Management experience preferred.
• Complies with all relevant professional standards of practice.
• Current CPR if applicable.
• TB questionnaire, PPD or chest x-ray if applicable.
• Current Health certificate (per contract or state regulation).
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

The RN Case Manager is responsible for coordinatingcontinuum of care activities for assigned patients and ensuring optimumutilization of resources, service delivery, and compliance with medical regime.
