The ideal candidate holds active RN licensure in Indiana, with 1+ year of case management experience preferred and CPR as applicable. You will work to meet regulatory requirements and deliver high-quality patient care.

Qualifications

• Current RN licensure in state practicing.
• At least one year of Case Management experience preferred.
• Complies with all relevant professional standards of practice.
• Current CPR if applicable.
• TB questionnaire, PPD or chest x-ray if applicable.
• Current Health certificate (per contract or state regulation).
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.

Responsibilities

• Coordinate continuum of care activities for assigned patients.
• Ensure optimum utilization of resources and service delivery.
• Ensure compliance with medical regime.
