Job summary

The Dimock Center's Askia program seeks a compassionate Case Manager to provide individualized care coordination, advocacy, resource navigation, and discharge planning for adult males in recovery.

You will build trusting relationships, coordinate housing, healthcare, employment, and benefits, and work with a multidisciplinary team to support recovery and community reintegration. Schedule is Mon–Fri 9:00am–5:00pm with occasional evenings and weekends.

Qualifications

  • Bachelor's degree in human services, psychology, sociology, health services, or related field.
  • At least one year of experience in a substance use treatment program.
  • Experience with homelessness or complex social needs preferred.

Responsibilities

  • Build trusting relationships and engage clients in treatment goals.
  • Meet with clients regularly to assess needs and coordinate services.
  • Develop individualized care plans with the treatment team.
  • Coordinate referrals to housing, healthcare, employment, and community resources.
  • Advocate for clients within health and human service systems.
  • Begin discharge and aftercare planning early.
  • Complete clinical and administrative documentation accurately.
