Job summary

Dimock Center invites applications for the Case Manager role, part of the clinical team providing ongoing care coordination and aftercare planning to individuals receiving residential services. You will handle advocacy, assessments, referrals, transport to appointments, discharge planning, and collaboration with counselors, physicians, and community partners to ensure seamless, patient‑centered care.

This position requires Bachelor’s in a human services field and at least one year in a substance

Qualifications

• Bachelor's Degree in human services, psychology, sociology, health services, or a related field.
• At least one year of experience working in a substance use program.

Responsibilities

• Demonstrate ability to establish and maintain engagement of client toward meeting treatment plan goals.
• Meet with clients weekly to provide care coordination and aftercare planning.
• Conduct assessments to identify clients' needs, strengths and barriers to attaining treatment plan goals.
• Conduct care planning in collaboration with clients and treatment team.
• Facilitate psycho-educational groups.
• Ensure that clients' whole-health and social determinants of health needs are identified and addressed.
• Conduct post-discharge follow-up for clients.
• Facilitate and carry out referrals and linkages to all service needs that clients have.
• Demonstrate professional and respectful boundaries with clients.
• Collaborate with all service providers to fully address the needs of clients.
• Provide transportation as needed to any appointments/meetings in the community.
• Complete all required documentation in a timely manner and in accordance with Joint Commission, Dimock and regulatory standards.
• Coordinate discharge and aftercare planning well in advance of discharge to ensure that clients receive a full continuum of care.
• Attend all staff meetings and trainings as assigned.
• Assist clinical team in crisis management.
• Return all phone calls to providers within 24 hours of receiving the call.
• Adhere to all organization, federal and state regulations, guidelines and/or contractual obligations.
• Maintain respectful, effective and professional working relationships with other Dimock staff.
• Demonstrate working knowledge of Dimock's continuum of care and facilitate appropriate referrals/contacts within the larger system.
• Demonstrate knowledge of and adherence to Dimock's Policies and Procedures.
• Perform other duties as assigned.
