Job summary

The Dimock Center is seeking a compassionate Case Manager to provide individualized care coordination, advocacy, and discharge planning for clients in recovery and community reintegration.

You will work within a clinical team to assess needs, coordinate services, and connect clients to housing, employment, benefits, and healthcare resources. Strong communication, organization, and commitment to person-centered care are essential.

Qualifications

• Bachelor's degree in human services, psychology, sociology, health services, or related field.
• At least one year of experience in a substance use treatment program.
• Experience with homelessness or complex social needs preferred.

Responsibilities

• Build and maintain trusting, professional relationships with clients.
• Meet with clients regularly to assess needs and coordinate services.
• Conduct comprehensive assessments to identify strengths, needs, barriers, and supports.
• Develop individualized care plans with clients and treatment team.
• Coordinate referrals to housing, employment, health, and community resources.
• Advocate for clients within health and human service systems.
• Address social determinants of health impacting treatment and recovery.
• Coordinate care with primary care and external partners.
• Assist with housing, benefits, and financial resource processes.
• Coordinate transportation and accompany clients to appointments and court.
• Facilitate psychoeducational and recovery groups.
• Discharge and aftercare planning to ensure continuity of care.
• Maintain confidential documentation per standards.
