Job summary

Northeast Recovery in Canton, MA is seeking a full-time Case Manager to support individuals and families through recovery. The role involves intake assessments, group and individual case management, treatment planning, and coordination with the care team.

You will work under the Program Coordinator, handle a caseload, maintain accurate records, and help clients access community resources. Preferred qualifications include LADC-2 and 3–5 years in behavioral health.

Qualifications

• Bachelor's Degree preferred
• LADC-2 Certification preferred
• 3–5 years of Case Management experience in a behavioral health setting

Responsibilities

• Conduct psycho-educational groups and individual case management as directed.
• Complete Intake Assessments.
• Conduct treatment team meetings and family sessions.
• Manage and handle assigned caseload, assessing clients' most urgent needs.
• Perform an in-depth mental or physical health analysis to develop a plan of action and set goals.
• Offer counseling in either group or individual settings.
• Consult with external agencies to provide support services and resources.
• Maintain comprehensive patient records of clients' progress.
• Prepare a comprehensive discharge plan based on client needs.
