Job summary

Basecamp Recovery Center is seeking a dedicated Case Manager in Whitehall/Columbus to link patients with housing, employment, and community services. You will advocate for access to needed supports and coordinate assessments, childcare, transportation, finances, and education within a supportive team.

You will maintain detailed progress notes, support treatment goals, and collaborate with clinicians while engaging with diverse clients.

Qualifications

• Bachelor’s degree in Counseling, Social Work, or related field preferred.
• Active licensure required: CDCA, LSW, LPC, LCDC-II, or LCDC-III.
• 3–5 years of experience working with homeless or underserved populations preferred.

Responsibilities

• Uphold the mission and values of Basecamp Recovery Center; maintain professional relationships with patients, staff, and community partners.
• Deliver high-quality substance use and mental health intervention services to patients.
• Develop and implement case management plans in collaboration with the clinical team, including assessments and service planning.
• Attend patient meetings, court appearances, school meetings, and other appointments as needed.
• Maintain confidential case records and documentation in accordance with standards.
