Job summary

Pyramid Healthcare in Allentown is seeking a full-time Case Manager to join our outpatient treatment team. You will coordinate client services, facilitate communication among providers and referral sources, support admissions and discharge planning, and help clients access resources to sustain long-term recovery.

The ideal candidate holds a state clinical license and a Master’s Degree in a human services field, actively pursuing CAADC or CADC licensure; a Bachelor’s or Associate’s degree is

Qualifications

• Active statewide clinical license or certification by a statewide board.
• Master’s Degree in a human services field and pursuing CAADC or state licensure.
• Bachelor’s or Associate’s in human services with pursuing certification (CADC/CAAC).
• RN licensure in Pennsylvania with relevant clinical experience.

Responsibilities

• Coordinate client engagement through appointments, treatment schedules, aftercare plans, meetings, and recovery support services.
• Support admissions processes with required consents, records, and referrals; maintain documentation in EHR.
• Serve as liaison between clients, referral sources, healthcare providers, and community partners.
• Develop and coordinate individualized aftercare plans in collaboration with the treatment team.
• Participate in multidisciplinary team meetings and ensure confidentiality and compliance.

Master's Degree in a human services field

Bachelor's Degree in a human services field

Associate's Degree in a human services field

High School Diploma or equivalent

Registered Nurse (RN) licensure in PA
