Job summary

Pyramid Healthcare is seeking a Case Coordinator to support client engagement and aftercare planning at the Martinsburg Inpatient location. Schedule is Monday through Friday, 8:00 am to 4:30 pm, with an hourly rate of $18–$20 based on education and experience.

Responsibilities include coordinating with treatment teams, referral sources, and outside providers; documenting contacts in Carelogic; and assisting with transportation needs and EOS appointments.

Qualifications

• High School Diploma required.
• Bachelor’s degree in a human services related field preferred.

Responsibilities

• Provide case coordination services and arrange transportation for clients.
• Conduct individual case coordination sessions and assist in developing aftercare plans.
• Assist with groups to support clinical services and weekly engagement.
• Collaborate with counselors and the treatment team; document contacts in the care system.
• Coordinate with client navigation for aftercare needs and EOS appointments.
• Participate in mandatory trainings and professional development.
• Other duties as assigned.

Case Coordinator

• Address: Pyramid Healthcare Martinsburg Inpatient
  • 64 Boy Scout Road, Hedgesville, WV
• Schedule: Monday through Friday 8:00 am - 4:30 pm
• Hourly Rate: $18-$20/hour (based on education/experience)

Essential Duties and Responsibilities:

• Coordination of client engagement through appointments, aftercare plans, schedules and meetings in collaboration with treatment team.
• Coordination and communication with referral sources and outside providers through consistent, professional communication, assessment of needs, and documentation of contact at admission and for the duration of treatment. **This may include communication or coordination for UR, legal, and employment needs for the client.
• Provides case coordination services for clients through coordinating transportation needs.
• Providing individual case coordination sessions. Assists in development of aftercare plan, providing referral and aftercare provider contact, addressing outside provider needs.
• Assists with groups to support clinical services provided, and supporting clients on a weekly basis to support engagement in the program.
• Participates in treatment team collaboration and maintains professional, collaborative communication with counselors and treatment team.
• Documentation of client, family, referral, and outside provider contacts in Carelogic in accordance with program timelines
• Collaboration with client navigation team for client aftercare needs.
• Provides onsite support for Enhanced Outpatient Sessions (EOS) appointments between clients and Client Navigation team.
• Participate in mandatory corporate training, program training and professional development.
• Other duties as assigned

Education, Licensure, & Experience

• High School Diploma required
• Bachelor’s degree in a human services related field preferred
