The role involves communication with referral sources and outside providers, documenting contacts in Carelogic, and supporting aftercare planning and client transportation as needed.

Qualifications

• High School Diploma required
• Bachelor’s degree in a human services related field preferred

Responsibilities

• Coordinate client engagement through appointments, aftercare plans, schedules and meetings with the treatment team.
• Communicate with referral sources and outside providers; assess needs and document care at admission and throughout treatment.
• Coordinate transportation needs for clients and provide case coordination sessions.
• Assist in developing aftercare plans; provide referrals and contact with aftercare providers.
