Job summary

Pinnacle Treatment Centers in State College, PA, seeks a dedicated Case Manager to support patients in our addiction treatment program. You will partner with the Clinical Director and Executive Director to coordinate care, develop treatment plans, and ensure seamless service delivery while maintaining confidentiality and regulatory compliance.

Requirements include a Bachelor's degree in Human Services, a valid driver's license, and willingness to travel up to 15%.

Qualifications

• Bachelor’s degree in Human Services or related field is required.
• Valid/Current Driver's License is required.
• Ability to travel up to 15% may be required.

Responsibilities

• Provide case management and care coordination for patients in treatment.
• Coordinate continuum of care activities and ensure optimum utilization of resources.
• Coordinate assessments, treatment planning, and crisis intervention services as needed.
• Coordinate services with family, community resources, and criminal justice systems as appropriate.
• Attend team meetings and maintain confidentiality and compliance with policies.
