Job summary

Calvary Healing Center in Phoenix is seeking a Case Manager to work under the Clinical Director, coordinating care and mentoring staff. You will oversee treatment plans, utilization review, discharge planning, and psychoeducation groups while collaborating with the healthcare team to ensure quality patient services.

The role requires a Bachelor's degree (Master's preferred) and at least 2 years of case management experience, plus valid fingerprint clearance and relevant trainings.

Qualifications

• Bachelor's degree required, Master's degree preferred.
• Minimum of 2 years of experience in case management related work.
• Valid Fingerprint Clearance Card
• Completion of BLS and First Aid Training within 30 days of employment.
• Completion of Verbal De-Escalation Training within 30 days of employment.

Responsibilities

• Coordinate and facilitate the effective delivery of quality services to Calvary patients.
• Responsible for treatment plans, utilization review, discharge planning, psychoeducation groups, and case management.
