Job summary

First Step House in Salt Lake City, Utah, is seeking a full-time REACH Case Manager to support clients involved in the criminal justice system and experiencing substance use challenges. You will conduct assessments, develop individualized case management plans, help with housing, employment, education, and connect clients with resources while collaborating with clinical teams and community partners.

Responsibilities

• Build relationships and support progress: complete client assessments and develop individualized case management plans.
• Promote stability and independence: help clients find housing, develop employment plans, and access education and healthcare.
• Provide hands-on case management: maintain a caseload of 50+ clients and support housing-related services.
• Teach and encourage growth: facilitate life-skills and psychoeducational classes.
• Connect clients with resources: coordinate access to employment services, healthcare, housing, and other programs.
• Collaborate across teams: coordinate with counselors, case workers, clinical staff, and partners.
• Coordinate recovery support services: manage voucher programs and recovery resources.
• Maintain accurate documentation: update case notes, billing, and reports.
• Support program outcomes: track data and prepare funding reports.
• Protect client privacy: follow HIPAA and confidentiality requirements.
• Contribute to program success: assist with hiring and participate in supervision.

The REACH program supports individuals involved in the criminal justice system who are at high risk for recidivism and experiencing moderate to severe substance use disorders. As a REACH Case Manager, you'll help clients build the skills, connections, and stability they need to move forward in recovery and in life.

What You'll Do

As a REACH Case Manager, you'll help clients identify their goals, overcome barriers, and connect with resources that support lasting change. Responsibilities include:

• Build relationships and support progress: Complete client assessments, develop individualized case management plans, and meet regularly with clients to review goals and progress.
• Promote stability and independence: Help clients find and maintain safe, affordable housing; develop employment and career plans; access education and healthcare; create budgets; and strengthen essential life skills.
• Provide hands-on case management: Maintain a caseload of 50+ clients, including providing housing-related case management and support to individuals experiencing homelessness.
• Teach and encourage growth: Facilitate life-skills and psychoeducational classes that address criminogenic risk factors and support positive decision-making.
• Connect clients with resources: Build relationships with community partners and coordinate access to employment services, education, healthcare, housing, and other support programs.
• Collaborate across teams: Work closely with counselors, case workers, clinical staff, legal partners, and outside agencies to coordinate services and support client success.
• Coordinate recovery support services: Manage the client voucher program and help individuals access resources that reinforce their recovery goals.
• Maintain accurate documentation: Complete case notes, client updates, billing information, treatment-related documentation, and required reports accurately and on time.
• Support program outcomes: Track data, prepare reports for funding sources, and use databases and software such as UWITS, Microsoft Word, and Excel.
• Protect client privacy: Obtain appropriate releases of information and follow HIPAA and applicable confidentiality requirements.
• Contribute to program success: Assist with hiring as needed, coordinate with community partners, and participate in regular supervision and performance discussions.
