Job summary

Orlando Rehabilitation Hospital seeks a Case Manager to guide patients through the continuum of care, coordinating clinical and financial aspects with physicians to drive timely, cost-effective, individualized services for diverse rehab patients including spinal cord and brain injuries.

You will work to align treatment plans with CMS guidelines, ensuring timely care within a collaborative, multidisciplinary team while connecting patients with vocational resources and community supports.

Qualifications

• Current state licensure as a Registered Nurse or MSW
• MSW without licensure acceptable
• Minimum of 3 years healthcare and clinical experience in a hospital setting
• Desired: 1 year hospital experience preferred
• Certification in case management preferred
• Knowledge of reimbursement systems preferred
• Excellent communication, negotiation, and conflict resolution skills
• Excellent verbal and written communication
• Strong organizational, time management and prioritization skills
• Strong analytical and critical thinking skills
• Detail-oriented, able to meet strict time frames

Job description

The Case Manager, in collaboration with the physician, provides individual program management for each patient to ensure the patient’s progression through the continuum of care in a manner that achieves the desired clinical and financial outcomes. Monitors and manages clinical and financial coordination of treatment plan of assigned patients to ensure timely, cost-effective, individualized service delivery. Works with rehabilitation patients with various disabilities including, but not limited to: spinal cord injury, brain injury, cerebrovascular accident, amputation, neurologic disorders, orthopedic conditions, and arthritis. Coordinates length of stay management within Medicare (CMS) guidelines and 60% compliance threshold. Provides community resources as appropriate including vocational rehabilitation resources.

Pay: Rate of pay is based on years of experience and qualifications.

Minimum Qualifications:

• Current state licensure as a Registered Nurse, Licensed Social Worker,
• MSW without licensure will be considered
• Minimum of 3 years healthcare and clinical experience in a hospital setting

Desired Qualifications:

• 1 years experience in a hospital setting preferred
• Certification in case management preferred.
