Job summary

The Connection in Torrington, CT is seeking a Case Manager for Primetime Clubhouse to support individuals facing housing instability. This full-time role requires building housing plans, coordinating services, and delivering coordinated case management with a focus on stability and client-centered goals.

Responsibilities include assessments, regular meetings, home visits, and referrals to community resources. 40 hours/week, first shift, with some flexibility to meet program needs.

Qualifications

• 5 years of related experience with Some post-secondary education or a Bachelor's degree with 2 years of related experience.
• Knowledge of CAN System and local housing opportunities for low-income adults.
• Experience with the homeless and SPMI populations.
• Valid Driver’s License.

Responsibilities

• Provide comprehensive case management services to individuals with mental illness and housing instability.
• Perform initial and ongoing assessments, and conduct regular case management meetings and home inspections.
• Coordinate and refer to community resources; develop service plans addressing housing stability and individual needs.
• Collaborate with clients to address strengths, needs, abilities, and preferences.

Position Summary

The Litchfield Next Steps Housing Case Manager provides comprehensive case management services to individuals with severe and persistent mental illness, co-occurring disorders, histories of homelessness and current housing instability. The Housing Case Manager provides initial and ongoing comprehensive assessments; regularly scheduled case management meetings and home inspections; coordination of and referral to community resources; and works collaboratively with clients on caseload to develop individual service plans in an effort to address housing stability and goals that reflect individual strengths, needs, abilities, and preferences.

Requirements

• Some post-secondary education is required with 5 years of related experience or a Bachelor’s Degree with 2 years of related experience
• Knowledge of CAN System and local housing opportunities for low-income adults.
• Experience working with the homeless and SPMI populations.
• Valid Driver’s License

Orientation

If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
