Job summary

The Litchfield Next Steps Housing Case Manager at The Connection, Inc. provides comprehensive case management for individuals facing severe mental illness and housing instability in Torrington.

Responsibilities include assessments, service planning, regular case management meetings, and coordination of community resources to address housing needs. A valid driver’s license and relevant experience are required.

Qualifications

• Formal case management experience required (or equivalent).
• Experience working with homeless populations and people with serious mental illness (SPMI).
• Knowledge of CAN system and local housing opportunities for low-income adults.

Program: Primetime ClubhousePosition: Case ManagerLocation: TorringtonSalary: $22.00/hourSchedule: Full Time, 40 hours, 1st shift, Monday-Friday, with flexibility to meet the needs of the program.Position Summary:The Litchfield Next Steps Housing Case Manager provides comprehensive case management services to individuals with severe and persistent mental illness, co-occurring disorders, histories of homelessness and current housing instability.
