Qualifications

• Bachelor’s degree in Psychology, Social Work, Human Services, Criminal Justice or related field.
• Two or more years of experience working with diverse populations facing substance use, mental health, and/or housing insecurities.
• Valid Colorado driver’s license with acceptable driving record.

Responsibilities

• Provide case management services to PREVAIL clients and connect them to social services.
• Interact with the LPD Community Action Team during field and office duties.
• Engage in outreach by phone and in person to assess needs and eligibility for services.
• Maintain up-to-date case notes and documentation in required systems.
• Participate in supervision and contribute to team problem‑solving and planning.
• Represent PREVAIL and Lakewood Police Department in community outreach meetings.
