Job summary

Mosaic is hiring a Person Centered Plan Case Manager in Beatrice, NE. You will manage a diverse caseload, coordinate annual assessments, and work with families and coordinators to ensure compliant admissions and high-quality care.

Qualifications

• 3+ years of hands-on experience working with individuals with intellectual/developmental disabilities.
• Deep knowledge of habilitative services and state regulations; behavior intervention techniques.
• QIDP certification (or state requirements), Med Aide 40, CPR/First Aid and Mandt certification required (if not present, may be obtained after hire).

Responsibilities

• Oversee a caseload of up to 22 individuals, creating individualized programs.
• Lead annual meetings and assessments (CFA, POM, SIS) and present to the Human Rights Committee.
• Collaborate with Home Supervisors, families, guardians, and Service Coordinators to drive admissions and ensure regulatory compliance.
• Coach direct care staff in behavior modification techniques and help develop innovative service methodologies.
• Maintain high standards with professional documentation and quality of care.
