Job summary

Mosaic Beatrice is seeking a Person Centered Plan Case Manager to lead service delivery for up to 22 individuals, blending advocacy with coordinated supports in a community setting.

You will oversee annual assessments (CFA, POM, SIS), collaborate with families, guardians, and service coordinators to manage admission dates, train staff in behavior modification, and maintain clear, professional documentation.

Qualifications

• Education: Bachelor's Degree in Psychology, Special Education, or a closely related field.
• Experience: At least 3 years working with individuals with intellectual/developmental disabilities.
• Certifications: QIDP, Med Aide 40, CPR/First Aid and Mandt certification (or obtain after hire).

Responsibilities

• Empower & Advocate: Oversee a diverse caseload of up to 22 individuals with person-centered programs.
• Lead Annual Meetings & Assessments: Coordinate CFA, POM, SIS, and annual reviews.
• Collaborate for Success: Work with families, guardians, and service coordinators to manage admissions and compliance.
• Coach & Innovate: Train direct care staff in behavior modification techniques and new service methods.
• Maintain High Standards: Ensure quality of care and clear documentation.
