Job summary

Mosaic Beatrice is seeking a Habilitative Coordinator-QIDP/Case Manager to empower and advocate for up to 22 individuals, blending hands-on advocacy with strategic coordination to support growth and independence.

This full-time role requires a Bachelor's degree in psychology or related field, 3+ years’ experience with IDD, QIDP and related certifications, and a commitment to our values of belonging, connection, faithfulness, and grit.

Qualifications

• Bachelor’s Degree in Psychology, Special Education, or related field.
• At least 3 years hands-on experience with individuals with intellectual/developmental disabilities.
• Deep knowledge of habilitative services, ICFs state regulations, and behavior intervention techniques.
• QIDP certification (or state requirements) and related certifications; willingness to obtain after hire.
• Strong critical thinking, problem-solving, written communication, and time-management skills.

Responsibilities

• Oversee a caseload of up to 22 individuals, creating dynamic, individual-centered programs that support personal growth and independence.
• Coordinate and present key assessments (CFA, POM, SIS), lead annual review meetings, and present before the Human Rights Committee.
• Build strong partnerships with Home Supervisors, families, guardians, and Service Coordinators to drive admission dates and maintain regulatory compliance.
• Train direct care staff in behavior modification techniques and help develop innovative service methodologies.
• Monitor the quality of care and maintain clear, professional documentation.

What You’ll Do

Requirements

What You Bring to the Team

• Education:REQUIRED -Bachelor’s Degree in Psychology, Special Education, or a closely related field


• Experience:At least 3 years of hands‑on experience working with individuals with intellectual/developmental disabilities.


• Expertise:Deep knowledge of habilitative services, Intermediate Care Facilities state regulations, and behavior intervention techniques.


• Certifications:QIDP certification (or state requirements), Med Aide 40, CPR/First Aid and Mandt certification required (if you do not have then required to obtain after hire).


• Core Competencies:Exceptional critical thinking, strong problem‑solving ability, excellent written communication, and structured time‑management skills.
