Job summary

Mosaic Beatrice is seeking a Person Centered Plan Case Manager to lead service delivery for individuals with intellectual and developmental disabilities. This full-time salaried role blends hands-on advocacy with strategic coordination to support personal growth and independence.

You will manage a caseload of up to 22 people, oversee annual assessments, collaborate with families and guardians, train direct care staff, and ensure regulatory compliance while maintaining clear, professional

Qualifications

• Education: Bachelor’s degree in Psychology, Special Education, or closely related field.
• Experience: At least 3 years working with individuals with intellectual/developmental disabilities.
• Expertise: Knowledge of habilitative services, ICF state regulations, and behavior intervention techniques.
• Certifications: QIDP (or state requirements), Med Aide 40, CPR/First Aid and Mandt certification (if not present, must obtain after hire).
• Core competencies include critical thinking, strong problem-solving ability, written communication, and time-management skills.

Responsibilities

• Oversee a diverse caseload of up to 22 individuals, creating dynamic, individual-centered programs that support personal growth and independence.
• Coordinate and present key assessments (CFA, POM, SIS), lead annual review meetings, and present before the Human Rights Committee.
• Build strong partnerships with Home Supervisors, families, guardians, and Service Coordinators to drive admission dates and ensure regulatory compliance.
• Train direct care staff in behavior modification techniques and help develop innovative service methodologies.
• Monitor the quality of care provided and maintain clear, professional documentation.

What You Bring to the Team

• Education: REQUIRED - Bachelor's Degree in Psychology, Special Education, or a closely related field
• Experience:At least 3 years of hands-on experience working with individuals with intellectual/developmental disabilities.
• Expertise:Deep knowledge of habilitative services, Intermediate Care Facilities state regulations, and behavior intervention techniques.
• Certifications:QIDP certification (or state requirements), Med Aide 40, CPR/First Aid and Mandt certification required (if you do not have then required to obtain after hire).
• Core Competencies:Exceptional critical thinking, strong problem-solving ability, excellent written communication, and structured time-management skills.
