Zusammenfassung

AbleLight is seeking a Case Manager- QIDP in Littleton, CO to join our team delivering person-centered services in an ICF residential setting. You will facilitate planning and implementation of individualized plans, ensuring access to medical, social, educational, and other supports for adults with intellectual disabilities.

The role requires a bachelor’s degree in human services or a medical field, four years of case management experience, and a valid driver’s license.

Qualifikationen

• Bachelor’s degree in human services or a medical field is required.
• Four years of case management experience required.
• Valid Driver's License required.
• Must pass a background check.
• Hybrid position with 2–3 days on site.

Aufgaben

• Facilitates planning, development, and implementation of person-centered plans by completing assessments and reviews.
• Assists people with Intellectual Disabilities in gaining access to required medical, social, educational, and other services.
• Provides or coordinates direct case management activities for adults with Intellectual Disabilities.
• Performs needs assessments, including written comprehensive assessments of strengths, weaknesses, needs, and desires.
• Leads case planning and participates in Plan of Care development and review within set time frames.
• Assists individuals and families in identifying and linking needed services and supports.
• Supports social network expansion by linking with relevant persons, groups, or agencies.
• Performs reassessment and follow-up at quarterly intervals and documents progress and revisions as needed.
• Monitors services to ensure needs are met and documents outcomes and referrals.
• Attends interdisciplinary and interagency meetings as required and maintains documentation for accountability and billing purposes.
