Responsibilities

• Facilitates planning, development, and implementation of person-centered plans by completing assessments and reports.
• Assists people with Intellectual Disabilities in accessing needed medical, social, educational, and other services.
• Provides or coordinates direct case management activities to adults with Intellectual Disabilities.
• Conducts needs assessments and develops comprehensive plans of care.
• Plans and reviews the Plan of Care with appropriate timelines.
• Identifies services and resources and links individuals to needed supports.
• Supports social networks by linking individuals with groups or agencies.
• Reassesses progress quarterly and documents outcomes.
• Monitors services and maintains accurate documentation for accountability.
