Responsibilities

• Facilitates planning, development, and implementation of person-centered plans by completing assessments and reviewing data.
• Assist people with Intellectual Disabilities in accessing medical, social, educational, and other services.
• Provide or coordinate direct case management activities for adults with Intellectual Disabilities.
• Conduct needs assessments including comprehensive evaluations of strengths, weaknesses, needs and desires.
• Develop and review the Plan of Care within appropriate time frames and coordinate services accordingly.
• Monitor services and document progress, referrals, and outcomes; participate in revisions as needed.
