Breaking Ground operates nearly 4,000 units of housing across New York City, along with housing in upstate New York and Connecticut. Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.
But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.
The Scatter Site program provides community-based, independent apartment housing located throughout New York City and case management services for clients who have experienced chronic homelessness, substance use, and mental health challenges.
Job Title: Case Manager, Permanent Housing
The Case Manager is responsible for providing support services to help individuals remain housed and to address needs such as benefits/entitlements, activities of daily living, substance abuse, and mental health services. The Case Manager conducts home visits and links the client with local services in the community, as well as coordinating group activities for their caseload. The Case Manager provides case management services to formerly homeless individuals, some with multiple disabilities, in order to help the individual transition and adjust to permanent housing.
