Job summary

Mental Health Association of Monmouth County, Inc. is seeking a PATH Case Manager to provide outreach and person-centered case management for individuals experiencing serious mental illness who are homeless or at risk of homelessness.

You will assess needs, coordinate services, and support progress toward housing stability, recovery, and independence, maintaining accurate HMIS records and collaborating with community partners to improve access to resources.

Qualifications

• Bachelor's degree in mental health, social work, psychology, or related field.
• Minimum of two years of relevant case management experience.
• Experience in homelessness services or behavioral health.
• Strong written and verbal communication skills.

Responsibilities

• Provide person-centered case management to assigned clients by assessing needs, coordinating services, monitoring progress, and updating individualized service plans as circumstances change.
• Conduct outreach to individuals experiencing homelessness or at risk of homelessness using trauma-informed engagement strategies to build trust and connect individuals with PATH services.
• Assist clients in accessing and navigating community resources, including housing, healthcare, behavioral health treatment, benefits, and other social services.
• Identify client strengths, barriers, and support systems to promote service engagement, recovery, housing stability, and independence.
• Complete and maintain documentation in client files and HMIS, including required documentation and referrals, per agency policies and regulations.
• Maintain current knowledge of each client’s circumstances and progress.
• Collaborate with clients to develop crisis plans with warning signs and emergency contacts.
