Job summary

Catholic Charities of the Archdiocese of St. Louis is seeking a Case Manager to work within our Pathways to Progress Program in Washington County, MO. You will provide comprehensive case management to individuals and families facing social and economic challenges.

You will assess needs, develop service plans, coordinate resources, and support clients toward self-sufficiency, while ensuring compliance with program guidelines and documenting progress.

Qualifications

• Bachelor's degree in Social Work, Human Services, Counseling, Psychology, Sociology, or a related field required; Master's degree preferred.
• Minimum of three (3) years of experience providing case management, social services, or related support.
• Valid driver's license (MO residents require Class E), personal vehicle, and proof of automobile insurance required.

Responsibilities

• Conduct client intake, assessment, and reassessment activities to identify needs, strengths, barriers, and eligibility for services, and facilitate referrals to appropriate program and community resources.
• Develop, implement, monitor, and update individualized service plans in partnership with clients.
• Maintain an assigned caseload and provide services in accordance with established program service delivery models.
• Connect clients with appropriate internal and external resources and coordinate services with community partners.
• Monitor client progress and provide coaching, advocacy, and support to help clients achieve service plan goals.
• Maintain accurate, timely, and complete case records, reports, and documentation in accordance with ministry, program, and grant requirements.
• Respond appropriately to client crises by assessing needs, providing support, coordinating resources, and following established ministry protocols and emergency procedures.
• Serve as a liaison with community organizations and service providers to strengthen referral networks and client resources.
• Perform other duties as assigned.
