Job summary

CarePlus NJ, Inc. seeks a dedicated Case Manager for our Maternal Wrap-Around Program (M-WRAP). You will support women who are pregnant or postpartum with substance use disorders, delivering intensive case management and linking families to housing, health, and employment resources.

You will coordinate care with multiple providers, document progress, and advocate for clients while addressing child welfare considerations and neonatal concerns. Strong trauma-informed practice is essential.

Qualifications

  • Bachelor's degree in health, psychology, counseling, social work, education, or related behavioral health field.
  • At least three years of experience with high-need families dealing with substance use and mental health disorders.
  • Active CADC or in process of obtaining CADC is required.
  • Training in trauma-informed services is required.

Responsibilities

  • Provide intensive case management and conduct comprehensive assessments across housing, finances, transportation, legal services, vocational tasks, health and behavioral health care, and family strengths/needs.
  • Coordinate services across multiple agencies and share information with consent.
  • Link clients to community resources and ensure access to needed services.
  • Maintain regular contact with clients and conduct home visits.
  • Educate partners and medical staff on reducing stigma around pregnant women with SUD.
  • Prepare families for birth and potential child welfare involvement; connect to pediatric care and early intervention.
  • Organize team meetings to coordinate care from pregnancy through postpartum.

Job Summary:

As a Case Manager, you will work in our Maternal Wrap-Around Program (M-WRAP) where you will assist women who are pregnant and/or postpartum with substance use disorders in supporting and strengthening their capacity to participate in health practices and maintain stable homes through a family-centered approach. You will be responsible for providing intensive and direct case management services to a client caseload and assisting women and families in providing resources and linkages to necessary services ranging from housing, health, and employment issues.

What You Will Do:
  • Provide client with a comprehensive case management assessment that includes life domains, such as housing, finances, transportation, legal services, vocational, employment, health and behavioral health care, and family strengths and needs.
  • Promote information sharing between all providers on your client's progress and challenges with the informed consent of each client in prenatal and nutritional care areas.
  • Link client to appropriate care and resources in the community, including resources that address specialized needs.
  • Coordinate needed interventions and services provided by multiple agencies.
  • Remain in regular contact with the client and conduct home visits.
  • Refer and increase connections to supportive services, and work with Perinatal Cooperatives/Risk Reduction Specialists in educating the medical community partners on the stigma around pregnant women with a SUD.
  • Prepare client for birthing, including potential for child welfare involvement, NAS if on MAT or using opioids, accessing pediatrician familiar with prenatal exposure, preparing for living and care arrangements for infant.
  • Provide support and link families with appropriate services such as early intervention, child services, and services accessible through the NJ Prevention System of Care.
  • Organize team meetings that bring all services together on behalf of the client from pregnancy through postpartum and maintain contact with service providers that include all important information and updates.
What We Need to See:
  • Bachelor's Degree from an accredited college or university in health, psychology, counseling, social work, education, or other behavioral health professions.
  • At least three (3) years of experience working with high-need families with substance use and mental health disorders. Knowledge of systems that provide services to women and their families, such as maternal health, early childhood intervention, and child welfare.
  • Must possess an active CADC or be in the process of obtaining their CADC required.
  • Training in trauma-informed services.
