Job summary

Dallas County is seeking a Case Manager to provide comprehensive services for various county programs. You will identify client needs, assess eligibility, and offer counseling, outreach, and referrals to ensure successful program participation.

Responsibilities include interviewing clients, verifying documents, preparing case plans, and coordinating with multiple departments and community partners. Strong communication skills and a valid Texas driver’s license are preferred.

Qualifications

• Bachelor's degree or equivalent in Behavioral Science or related field.
• Prefers experience in intake, assessment, case planning and evaluation.
• Knowledge of community resources and how to access them.
• Strong communication, both verbal and written, with ability to build relationships.

Responsibilities

• Interviews clients, collects documentation and verifies information to assess needs and eligibility for programs.
• Analyzes documentation to ensure eligibility and comply with program requirements and laws.
• Provides counseling, referrals and follow-up services to clients.
• Prepares case plans, summaries, requests and reports.
• Coordinates with other departments, agencies and community groups to deliver services.
• Maintains knowledge of applicable policies, procedures and laws.
• Attends meetings and trainings to improve service delivery.
• Performs other duties as assigned.
