ICL in New York seeks a dedicated professional to provide direct services to individuals with mental disabilities in their homes. You will assess needs, participate in treatment planning, and offer compassionate counseling and daily living skills support.

You will document progress, maintain case records, and coordinate with a multidisciplinary team to ensure safe, effective care. Travel to consumer residences and crisis response are part of the role according to program policies.

Formación

• Bachelor's degree or associate degree.
• Two years' direct services experience with mental disabilities or related human services.
• One year practicum in mental disabilities (e.g., BSW) required.
• Associates degree plus 3 years above experience.

Responsabilidades

• Travel to consumers' residences to provide supportive services on a schedule established by the program.
• Develop and maintain consumer treatment plans with quarterly reviews.
• Document progress and maintain case records for each resident/consumer.
• Provide crisis assessment and nonverbal/ verbal crisis intervention.
• Explain clinical services and assist in daily living skill development.
• Coordinate discharge planning and community resource referrals.
