Institute for Community Living Inc. (ICL) in Brooklyn, NY seeks a dedicated professional to support individuals living in independent apartments within the community. You will help clients access medical, social, psychosocial, educational, financial, vocational, and other services, with crisis intervention as needed.

The role requires collaboration with a Program Director and a team to promote independence and community integration, with travel to residences and proactive case management.

Formación

• Bachelor's or Associate's Degree required.
• Experience providing direct services to individuals with mental disabilities or other relevant human services.
• Work eligibility to work in the U.S.

Responsabilidades

• Travel to consumer residences or apartments to provide supportive services as scheduled by the program.
• Maintain consumer treatment plans with quarterly reviews and required forms.
• Develop measurable service plans that maximize rehabilitative abilities.
• Provide 1:1 or group counseling to assist with attitude and behavior changes.
• Disseminate resource information and coordinate access to services.
• Utilize cognitive-behavioral techniques to support service plan goals.
• Assist in discharge planning and coordination with other team members.
• Explain available clinical services and everyday living skills to consumers.
• Conduct crisis assessments and document incidents as required.
• Accompany consumers to medical or community appointments as needed.
