Gosnold is seeking a Case Manager for BHJI/CSP in Centerville. The role requires a bachelor’s degree in a related field and at least two years of mental health/addiction experience.

You will manage a caseload, perform assessments, and connect clients with community resources while maintaining compliant records. The position involves working on-site, 40 hours per week, with pay in the $22–$23 per hour range and benefits typical of full-time employment.

Qualifikationen

• Bachelor’s degree required in counseling, psychology, social work, nursing, or related discipline.
• Two years of either professional or lived experience in mental health and addiction.
• Comfort engaging with individuals involved in the justice system and working in correctional settings.

Aufgaben

• Conduct patient interviews and basic assessments to engage patients into treatment and continuing care planning.
• Develop case management service plans and track progress in CRM systems.
• Coordinate with community resources to support recovery and transportation needs.
• Maintain patient records in line with Gosnold standards and regulatory requirements.
• Collaborate with multidisciplinary teams and participate in supervision and trainings.
