Gosnold, Inc. in Wareham, MA is seeking a BH-JI Navigator (40 hours) to support individuals with mental health and addiction recovery through assessments, case management, and treatment planning.

The role requires collaboration with correctional facilities and community resources, managing a caseload of about 10-15 patients, and documenting progress. Qualifications include a bachelor's degree in counseling, psychology, social work, nursing or related field and at least two years of experience in

Qualifications

• Education: Bachelor's degree in counseling, psychology, social work, nursing or related discipline preferred; two years experience may substitute.
• Experience: Two years in mental health and addiction services.
• Skills: Interpersonal, organizational, self-directed, and proficient with web-based EHRs.
• Certificates: Valid clean Driver’s License.

Responsibilities

• Conduct patient interviews, complete basic assessments, and initiate treatment planning.
• Manage caseload of 10–15 patients and track tasks in CRM.
• Coordinate with community resources and Houses of Correction.
• Maintain patient records per Gosnold standards.
• Collaborate with multidisciplinary team.
