Job summary

Gosnold, Inc. is seeking a BH-JI Navigator in Wareham, MA to support patients through the mental health and addiction recovery process. The role requires coordinating with community resources, conducting initial interviews, and developing care plans for a caseload of 10–15 patients.

You will work within a multidisciplinary team, document in EHR systems, and participate in supervision and team meetings. A valid driver’s license and a willingness to travel to various correctional facilities are

Qualifications

• Bachelor’s degree in counseling, psychology, social work, nursing or related discipline preferred.
• Two years of experience in mental health/addiction may substitute academic requirements.
• Strong interpersonal and organizational skills; proficient with web-based EHR.

Responsibilities

• Interview patients and complete basic assessments and engage patients into treatment and continuing care planning.
• Develop case management service plans.
• Coordinate with community resources and services; work with a multidisciplinary team.
• Maintain patient records in Gosnold standards; ensure compliance with regulations.
• Take part in supervision, training, and team meetings; outreach using own transportation.
