Job summary

Pittsburgh Comprehensive Treatment Center (CTC), part of Acadia Healthcare's Comprehensive Treatment Centers, seeks a Case Manager for our Outpatient MAT OTP program.

The role develops and coordinates patient care plans, collects clinical data, and provides input to the treatment team. This full-time position runs Monday–Friday 6:00 AM–2:30 PM and is grant-funded with renewal potential. Eligible positions may qualify for HRSA loan forgiveness.

Qualifications

• Bachelor’s degree in Social Work, Counseling or related Human Service field.
• 1-2 years of experience working in behavioral health setting. Experience with the population served by the clinic preferred.
• Previous experience in case management and/or utilization review preferred.

Responsibilities

• Provides quality care through developing, implementing, managing, and evaluating patient care plans;
• Collects clinical data, correlates this clinical data, and provides clinical input to the treatment team;
• Conveys medical criteria and clinical information between the insurance provider and treatment team as warranted;
• May also correlate clinical data for the business office as required;
• Providing case management and care coordination services for patients in the clinic;
• Responsible for coordinating the continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements;
• May also be responsible for conducting individual, and family educational sessions, various duties related to coordinating treatment and discharge activities, as needed;
• Coordinates in a timely manner, issues or activities relevant to the team, and/or to the team’s relationship within the facility or treatment milieu; and,
• Perform other duties as assigned.
