Westbrook Health Services seeks a dedicated professional to support clients in the Medication Assisted Treatment (MAT) program, coordinating connections to substance abuse services per their treatment plans. The role emphasizes assessment, linkage, advocacy, and crisis response within HIPAA guidelines.

The ideal candidate holds a Bachelor’s degree in Human Services, with strong teamwork, communication, and professional ethics.

Qualifications

• Bachelor’s degree in a Human Services field is required.
• Must have a valid driver’s license and automobile insurance.
• Must pass Criminal Investigation Background and DHHR background checks and drug screen.

Responsibilities

• Assist clients in MAT program and connect to substance abuse services.
• Develop and review treatment plans for each client.
• Advocate for and assist clients in obtaining needed services.
• Provide crisis support and counseling within scope.
• Disseminate information to communities about substance abuse.
• Conduct outreach and strategic planning for MAT services.
• Maintain confidentiality in accordance with HIPAA and policies.
