Qualifications

• Bachelor’s Degree in Nursing or Social Work required.
• RN license preferred; LVN/LPN acceptable.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs and benefits.
• CCM certification a plus.
• Must maintain continuing education/licensing.

Responsibilities

• Communicate regularly with residents and families about Plans of Care and treatment.
• Attend morning meetings to facilitate team communication.
• Coordinate care with physicians, payers, and facilities.
• Oversee admissions and discharge activities.
• Perform utilization review to ensure cost-effective care.
• Document accurately and timely.
