Job summary

Grand Haven Hospital (part of Trinity Health) is seeking a Case Manager LMSW to coordinate discharge planning and collaborate with physicians, care teams, and families. The role is part-time with rotating shifts in Grand Haven, Michigan.

The LMSW must hold a Master of Social Work and a Michigan social work license. Experience in discharge planning and community resource navigation is preferred, with documentation in the EMR and strong crisis intervention skills."

Qualifications

• Masters degree in social work required.
• State of Michigan social work license required.
• Prior employment experience in a related area preferred.

Responsibilities

• Identify and coordinate discharge planning with patients and a multidisciplinary team.
• Recognize and monitor barriers to safe discharge planning and intervene.
• Navigate community resources and serve as liaison for services.
• Perform conflict resolution and crisis intervention when appropriate.
• Document interventions, assessments and discharge planning in the EMR.
