Job summary

Grand Haven Hospital (part of Trinity Health) is seeking a Case Manager LMSW to coordinate discharge planning and collaborate with physicians, care teams, and families. The role is part-time with rotating shifts in Grand Haven, Michigan.

The LMSW must hold a Master of Social Work and a Michigan social work license. Experience in discharge planning and community resource navigation is preferred, with documentation in the EMR and strong crisis intervention skills."

Responsibilities

Position Purpose

Case Manager LMSW I to work in the Case Management Department at Grand Haven Hospital.

What You Will Need

• Masters degree in social work required
• Prior employment experience in a related area preferred
• State of Michigan, social work license required

What You Will Do

• Identify, coordinate communicate and participate in discharge planning with patients and a multidisciplinary team
• Recognize and monitor potential barriers to safe discharge planning and intervene as appropriate
• Navigate community resources as well as serve as a reference and liaison for community resources and service.
• Perform conflict resolution and crisis intervention when and where appropriate
• Accurately document interventions, assessments, and discharge planning in the electronic medical record
