Job summary

Henry Ford Wyandotte Hospital is seeking a Part Time Case Manager to support a patient-centered care team in identifying, assessing, planning and coordinating care to meet patient needs. The role emphasizes advocacy, safe discharge planning and collaboration across care settings.

The ideal candidate holds an RN license or LMSW in Michigan with 3+ years of clinical experience and strong communication, customer service and problem-solving skills to guide patients and families through complex care

Qualifications

• RN or LMSW with Michigan license; CCM/ACM preferred.
• Bachelor's in nursing or Master of Social Work; 3+ years clinical experience.
• Excellent verbal and written communication; strong customer service.
• Experience in discharge planning, home health, rehab, or managed care preferred.

Responsibilities

• Identify, assess, plan, facilitate, coordinate and advocate for patient needs.
• Support discharge planning and safe transitions between care settings.
• Collaborate with care teams to promote safety, quality and cost-effective outcomes.
• Document and communicate with stakeholders across internal and external teams.
