Job summary

PAM Health is seeking a Case Manager in Ohio to coordinate care for a designated patient population within an interdisciplinary team. You will review inpatient admissions to ensure appropriate resource use and determine discharge needs, coordinating the discharge plan accordingly.

You will work to achieve desired financial and quality outcomes, collaborating with payors and healthcare providers. This role supports ongoing professional development and offers comprehensive benefits for full-time

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Current BLS certification required.
• Disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.

Responsibilities

• Coordinate care for a designated patient population in collaboration with the interdisciplinary team.
• Review inpatient admission records to ensure appropriate level of care and utilization.
• Identify discharge needs and coordinate discharge planning.
• Maintain and enhance payor relationships and support quality and financial outcomes.

Education

State licensure preferred

BLS certification

Professional disciplines: OT, PT, RN, Respiratory Therapist, SLP, or Social Worker
