Job summary

Family Service of Roanoke Valley in Roanoke, VA seeks a Case Manager to serve as the first contact for counseling services and provide ongoing case management and advocacy for adults under private guardianship. You will handle intake, scheduling, insurance verification, and record-keeping while coordinating with programs across the community.

Responsibilities include person-centered case management, assessments, care planning, advocacy in hospitals and nursing facilities, benefits enrollment

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology or related field.
• At least 1 year of experience in social services, behavioral health, or case management.
• Strong communication, organizational and advocacy skills.
• Ability to work independently with a diverse caseload and sound judgment.
• Comfortable working with vulnerable populations and travel across the Roanoke Valley.

Job description

Family Service of Roanoke Valley (FSRV) is hiring a Case Manager to fill a dual-role position: serving as the first point of contact for individuals seeking counseling services, and providing ongoing case management and advocacy for adults under FSRV private guardianship.

Counseling Intake

• Answer and assess incoming service requests by phone, in person, and through referrals
• Schedule appointments, verify insurance, and connect clients to the right programs or community resources
• Maintain intake records, waitlists, and referral documentation
• Respond to crisis situations and follow agency protocols for safety and mandated reporting

Guardianship Case Management

• Provide person-centered case management for adults under FSRV guardianship, consistent with Virginia DARS standards
• Conduct client assessments and develop individualized care plans; maintain monthly face-to-face contact
• Advocate for clients in hospitals, nursing homes, assisted living, and community settings
• Assist clients with benefits enrollment (Medicaid, Medicare, etc.) and attend court proceedings as required
• Maintain accurate, timely records and provide on-call support as needed

What We’re Looking For

• Bachelor’s degree in Social Work, Human Services, Psychology, or a related field (relevant experience may be considered)
• At least 1 year of experience in social services, behavioral health, case management, or a related setting
• Strong communication, organizational, and advocacy skills
• Sound judgment and the ability to work independently with a diverse caseload
• Comfortable working with vulnerable populations including older adults, individuals with disabilities, and those with mental health needs
• Valid driver’s license, reliable vehicle, and willingness to travel throughout the Roanoke Valley service area
