We are offering a great opportunity for a full-time Case Manager-Justice Services for our Community and Crisis Department.
This position serves justice-involved transitional-age youth and adults by providing intensive community-based case management and transition coordination services for those preparing for release from incarceration and reintegrating into the community. The target population for this service is individuals in incarcerated settings and those assigned to specialty docket/diversion programming. The position is responsible for reducing barriers to care, minimizing service fragmentation, and connecting individuals to resources that support stability, recovery, wellness, and long-term independence. The Case Manager plays a critical role in care coordination, advocacy, discharge planning, transition support, and monitoring service engagement throughout the re-entry process. This position requires a highly collaborative, trauma-informed, person-centered, recovery-oriented, and strengths-based approach, with strong knowledge of justice systems, behavioral health systems, and community resources.
May be required to travel within the Greater Reach CSB catchment area.
Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection. Our Agency maintains a drug-free workplace.
