The role involves creating service plans, coordinating with community resources, and facilitating programs and groups to promote self-determination and stable housing.

Responsibilities

• Provide orientation to the facility and explain Living Agreement rules.
• Assess individuals to develop housing goals based on needs and strengths.
• Design and carry out a Service Plan for each household.
• Meet biweekly to monitor progress and document progress toward goals.
