Catholic Charities Brooklyn and Queens, Inc. is seeking a Case Manager III to serve clients at risk of homelessness and those who are homeless. The role involves comprehensive support services, including assessment, crisis intervention, intake, and financial assistance.

The position includes supervising interns, coordinating referrals, conducting home visits, and maintaining detailed case records. Requires a master's degree, Spanish language skills preferred, and willingness to work evenings and

Formación

• MS/MA degree in social service related field is required.
• Experience providing direct services to at-risk populations or individuals/families previously in shelter preferred.
• Bilingual skills (Spanish) a plus.
• Ability to work flexible hours, including weekends, evenings according to program needs.
• Able to conduct home/field visits and travel throughout the five boroughs as needed.
• Frequently lifts and/or moves up to 10 pounds.

Responsabilidades

• Supervise bachelor level interns.
• Provide case management services, assessment and intake to determine eligibility for services.
• Advocacy, linkage, and escort services to prevent homelessness.
• Provide regular visits and contact to ensure case progress.
• Maintain case records and input data into databases.
• Assess needs of the consumer/family including basic needs (food, clothing, shelter).
• Follow-up on referrals and respond to all referrals.
