Job summary

Telecare is hiring a Case Manager II in California to provide direct support to individuals with mental health needs. You will work under supervision as the primary case manager to help clients live as independently as possible in their chosen community.

The role involves coordinating services, transportation, and supports within the SHINE FSP program, with a schedule Mon–Fri 8:30 am–5:00 pm and a starting wage range of $19.28–$23.24 per hour. Geographic differentials apply based on location.

Qualifications

• High School Diploma or GED or higher; two years of direct mental health or community services; or an Associates/Bachelor’s in related field.
• Valid driver’s license with personal vehicle insurance in your name.
• Willingness to use personal vehicle to transport clients, with mileage reimbursed per IRS rate.

Responsibilities

• Under supervision, serve as primary case manager for individuals with mental illness and co-occurring disorders to support independent living.
