Job summary

Telecare Corporation in Marysville, CA is seeking a Case Manager II to provide direct case management support under supervision. You will assist individuals with mental illness to live independently in their community, coordinating services and supports through the SHINE FSP program.

Mon-Fri, 8:30 am–5:00 pm, with a starting wage range of $19.28–$23.24 per hour. GEO-based pay differentials apply, and starting pay reflects experience and location.

Qualifications

• High School Graduation or GED equivalent with 2 years direct mental health/community services experience; or an Associates degree with 1 year experience; or a Bachelor's in Social Services.
• Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use your personal vehicle to drive clients to appointments and groups, with weekly mileage reimbursement at the IRS rate.

Responsibilities

• Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and co-occurring disorders, helping them live as independently as possible in their chosen community.
