Job summary

The Salvation Army of Dane County is seeking a Case Manager II to assess client needs, develop and monitor service plans, and connect clients with housing and community resources. You will document progress, coordinate with partner agencies, and support ongoing case reviews.

Qualifications include a Bachelor’s degree in a relevant field, three years of social work experience, and a valid Wisconsin driver’s license. Community outreach and client advocacy are key facets of the role.

Qualifications

• Bachelor’s degree in social work, human services, psychology, sociology, or criminal justice.
• Three years social work experience; management experience helpful.
• Valid Wisconsin Driver’s license with approval to drive from The Salvation Army’s insurance.

Responsibilities

• Conducts casework interviews to determine program eligibility and status.
• Assesses clients' psychosocial, economic, and physical needs to plan services.
• Assist housing search, placement, and stability activities.
• Develops and monitors client case plans; refer to community resources.
• Input data into HMIS and maintain case notes.
• Attend meetings with other providers and participate in case reviews.
• Provide training to new case managers.
