Job summary

The Salvation Army of Dane County is seeking a Case Manager II to assess client needs, develop service plans, and connect clients with housing and community resources. You will document cases, participate in team reviews, and train junior staff while coordinating services with partner agencies.

Education includes a Bachelor's in social work or related field; three years of social work experience is expected, with a valid Wisconsin driver’s license.

Qualifications

• Bachelor’s degree in social work, human services, psychology, sociology, or criminal justice is required.
• Three years of social work experience; management experience is helpful.
• Valid Wisconsin driver's license with approval to drive from The Salvation Army’s insurance.

Responsibilities

• Conducts casework interviews with clients to determine eligibility and status.
• Assesses psychosocial, economic, and physical needs to plan service delivery.
• Assists clients with housing search, placement, and stability.
• Develops and implements individualized case plans.
• Refers clients to community resources and advocates for them.
• Prepares case notes, writes reports, and enters data into HMIS.
• Reviews cases regularly and participates in consultations.
• Provides training to new case managers.
• Attends meetings with community partners.
• Conducts routine home visits and accompanies peers when needed.
