Position: Case Manager

Responsibilities
- Promotes optimal health for individuals through routine, preventive, and emergency health care. Ensures that work environments and Life Enhancement activities are safe, secure, and where individuals are free from harm.
- Provides supervision, training and coaching across all settings including Life Enhancement Program activities, to increase the individual’s self-sufficiency and independence, and to meet the needs and expectations of employers, volunteer sites, families and individuals supported. Provides support to employers as necessary and ensures quality and consistency of work performed by these individuals.
- Provides education, training, counseling, guidance and physical assistance when necessary to achieve the individual’s goals in employment, education, social growth, health and wellness, communication skills, community mobility and volunteerism.
- Develops, writes and presents annual Individual Support Plans (ISP’s) and the subsequent support paperwork in consultation with supervisor. Advocates and ensures that ISP plans are implemented to achieve the individual’s goals through employment and the Life Enhancement Program.
- Performs case management duties including documentation of training and activities, maintenance of individual confidential files on assigned caseload, review and submission of all required paperwork to supervisor according to agency procedures. Attends regular meetings as necessary.
- Maintains flexibility regarding necessary changes in work schedule and tasks as assigned by supervisor to cover the needs of the individual supported.
- Maintains regular communication with individuals and all ISP team members and other persons significant to the individual served (e.g. family, guardians, advocates) to facilitate consistency of programming and support. Acts as liaison between Lifeworks, employers, volunteer sites and community supported sites to ensure that individuals are meeting all expectations in the community.
- Maintains confidentiality and professional demeanor with regards to all individuals and staff members.
- Provides transportation to and from various sites in the community by either Lifeworks or personal vehicles as necessary. Staff are reimbursed at the agency mileage rate for any use of personal vehicle within the guidelines established by Lifeworks.
- Demonstrates good computer skills and ability to learn to utilize the agency’s computer programs for documentation of necessary data.
- Other duties and/or assignments to work/program locations based on the needs of the company.
Requirements
- Experience working with individuals with ID/DD
- Either a Bachelor’s degree or an associate degree in a related field with two years of experience, or an HS diploma and five years of related work experience in Human Services.
- Spanish speaking is a plus
- Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks
- Valid Driver’s License required
- Valid MAP certification is preferred
- Excellent verbal and written communication skills
